Service Terms & Conditions

Effective August 26, 2026. These terms govern every tattoo, piercing and laser service performed at InkSquad Studio. Read them together with the Service Liability & Release form, then complete and sign the shop's liability form, online or at the shop, before your appointment.

1. What these terms cover

These Service Terms & Conditions ("Terms") apply to all tattoo, body piercing and laser services performed at InkSquad Studio, 5118 W Camelback Rd, Phoenix, AZ 85031 (the "Studio"). By booking an appointment, paying a deposit, or signing the Client Acknowledgment form, you agree to these Terms. They are posted on this website so you can read them at home, before you are sitting in the chair. The separate website Terms & Conditions cover use of this website itself.

2. Age and identification

Arizona law (A.R.S. § 13-3721) makes it a criminal offense to tattoo, brand, scarify, implant or pierce a person under eighteen years of age unless that person is accompanied by a parent or legal guardian who is physically present for the procedure.

  • Tattoos and laser services: Studio policy is adults only. You must be eighteen or older and present a valid government-issued photo ID at every visit. No exceptions.
  • Piercings for minors: a parent or legal guardian must attend and remain physically present for the entire procedure. Both the minor and the guardian must present identification, and the guardian must sign the consent paperwork. Some placements are not offered to minors at all; the piercer's decision is final.
  • The Studio keeps consent and identification records with your paperwork. If you cannot or will not show ID, we cannot perform the service.

3. Consent paperwork comes first

Before any procedure you will complete and sign the shop's liability form, which confirms that you have read these Terms and the Service Liability & Release form. You can complete it online before you arrive or at the shop. No signed paperwork, no procedure. If anything in these documents is unclear, ask before you sign; our staff will walk you through it.

4. Health and medical disclosure

You agree to answer the health questions on the consent form truthfully and completely. That includes, without limitation: pregnancy or nursing; diabetes; hemophilia or any bleeding or clotting disorder; use of blood-thinning medication; heart conditions; epilepsy or seizure disorders; immune system disorders; skin conditions at or near the service area; a history of keloid scarring; allergies to latex, adhesives, pigments, metals, numbing agents or antiseptics; recent use of alcohol or drugs; and medications that affect skin or healing, such as isotretinoin or photosensitizing drugs.

The Studio relies on your answers to decide whether and how to perform the service and may decline or modify a service based on what you disclose. Withholding or misstating health information is at your own risk. Our artists, piercers and technicians are not medical professionals; nothing they tell you is medical advice. If you have any doubt about whether a procedure is safe for you, ask your physician first.

5. Inherent risks and assumption of risk

Tattooing, piercing and laser work affect living skin. Even when performed properly, with sterile single-use needles and equipment prepared to industry standards, these procedures carry inherent risks. They include infection, allergic reaction, swelling, bruising, scarring, keloid formation, pigment migration or blowout, jewelry migration or rejection, changes in skin pigmentation, blistering, and the possibility that the healed result differs from what you pictured. The Service Liability & Release form describes these risks service by service.

By proceeding with a service you confirm that you have read that form, that you have had the chance to ask questions, and that you voluntarily accept the inherent risks of the procedure you have chosen.

6. Release and limitation of liability

To the fullest extent permitted by Arizona law, you release InkSquad Studio, its owners, artists, piercers, technicians and staff from claims, demands and damages arising out of the inherent risks of a properly performed procedure, your own failure to follow written aftercare instructions, or your failure to disclose accurate health information. To the same extent, the Studio is not liable for indirect, incidental or consequential damages arising from any service.

Nothing in these Terms limits or excludes liability that cannot lawfully be limited or excluded under Arizona law, including liability for gross negligence or willful misconduct.

7. Bookings, deposits and cancellations

  • Consultations are free and carry no obligation.
  • A deposit may be required to hold an appointment and reserve drawing time. Where a deposit is taken it is applied to the final cost of the work.
  • Deposits are non-refundable if you miss the appointment or cancel inside the notice window. The current notice window is confirmed when the deposit is taken.
  • Rescheduling with proper notice moves your deposit to the new date. Repeated rescheduling may forfeit the deposit.
  • Arriving significantly late may be treated as a missed appointment at the artist's discretion.

8. Pricing and payment

Quotes reflect the design, size and placement discussed at consultation. Changing any of those changes the price. Shop minimums may apply. Payment is due at the end of each session. Completed work is not refundable; tattooing, piercing and laser treatment are services performed on your body and cannot be returned. Concerns about healed results are handled under the touch-up policy in section 11.

9. Right to refuse service

Our artists, piercers and technicians may decline any request at their professional discretion, including designs, placements or jewelry that in their judgment will not heal well, will not age well, or are unsafe for the client. The Studio will not produce hateful or extremist imagery. Clients who arrive intoxicated or impaired will be asked to rebook; alcohol and many drugs also thin the blood and compromise healing. Abusive behavior toward staff or other clients ends the appointment.

10. During your appointment

  • Eat a real meal within a few hours before your session and come well hydrated.
  • Follow your artist's or piercer's instructions during the procedure, including staying still when asked.
  • Guests may be limited for space and hygiene reasons. Children who are not receiving a service should not be brought to appointments.
  • Do not touch equipment, needles, jewelry stock or work surfaces.

11. Aftercare, healing and touch-ups

You receive written aftercare instructions before you leave, and you agree to follow them. Healing outcomes depend heavily on aftercare compliance along with individual skin, sun exposure, medication and general health, all of which are outside the Studio's control. Touch-ups on our own tattoo work are handled at the shop; ask your artist about the current policy for your piece. Work done elsewhere is assessed case by case. If you believe a piercing or tattoo is not healing normally, contact the Studio promptly, and seek medical attention for anything that looks like an infection.

12. Piercing jewelry

Initial piercings are performed with implant-grade jewelry selected by the piercer for the placement and your anatomy. The piercer may decline to install jewelry purchased elsewhere. For hygiene reasons, jewelry cannot be returned or exchanged once the packaging is opened or the piece has been worn.

13. Laser services

Laser services are performed in accordance with Arizona law governing laser procedures. A consultation and patch assessment come before treatment. You must wear the eye protection provided, disclose photosensitizing medications, and follow pre- and post-treatment instructions, including sun avoidance on the treated area. Tattoo removal and lightening results vary between individuals and between inks; the Studio does not guarantee complete clearance or any specific number of sessions.

14. Photographs of your procedure

With your written permission on the consent form, the Studio may photograph finished work and use the images in its portfolio, on this website and on social media. Saying no is always an option and never affects your service.

15. Governing law and disputes

These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-law rules. If a dispute arises, both sides agree to first attempt to resolve it directly and in good faith. Any claim that cannot be resolved that way shall be brought in the state courts located in Maricopa County, Arizona, and you consent to venue and personal jurisdiction there.

16. General terms

  • Severability: if any provision of these Terms is held unenforceable, the rest remain in full force.
  • Entire agreement: these Terms, the Service Liability & Release form and the consent paperwork you sign at the Studio together form the whole agreement about your service.
  • Changes: the Studio may update these Terms by posting a revised version on this page with a new effective date. The version in effect on the date of your service is the one that applies to it.
  • No waiver: the Studio's decision not to enforce a provision on one occasion does not waive it for the future.
