Service Liability & Release Form

Effective August 26, 2026. This form is posted here so you can read it before your appointment. Read the statements below, then complete and sign the form online further down this page, or at the shop when you arrive. It applies together with the Service Terms & Conditions.

Read this as your own statement

Each numbered paragraph below is a statement you are making to InkSquad Studio, 5118 W Camelback Rd, Phoenix, AZ 85031 (the "Studio"). By signing the liability form, online below or at the shop, you adopt every statement that applies to the service you are receiving.

All services

  1. I am voluntarily receiving this service. Nobody has pressured me into it, and I am sober and of clear mind.
  2. I am eighteen or older and have shown valid government-issued photo ID, or I am a minor receiving a piercing with my parent or legal guardian physically present, as Arizona law (A.R.S. § 13-3721) requires.
  3. I have answered the health questions on the consent form truthfully and completely, including pregnancy or nursing, diabetes, bleeding or clotting disorders, blood-thinning medication, heart conditions, seizure disorders, immune disorders, skin conditions, keloid history, allergies, and medications that affect skin or healing. I understand the Studio relies on my answers.
  4. I understand that any procedure that affects the skin carries a risk of infection, allergic reaction, swelling, bruising, scarring and other complications even when performed properly with sterile, single-use equipment, and that my own biology, aftercare and habits strongly affect the result.
  5. I have received, read and agree to follow the written aftercare instructions, and I understand that the Studio is not responsible for problems caused by my failure to follow them, by contamination after I leave, or by health conditions I did not disclose.
  6. I understand that Studio staff are not medical professionals, that nothing they say is medical advice, and that I should seek medical attention for signs of infection or any concerning reaction.
  7. I understand that completed services are not refundable and that healed results can differ from how the work looks the day it is done.

Tattoo services

  1. I have reviewed and approved the design, spelling, placement and colors of my tattoo before the needle touches skin. Approved custom artwork reflects my instructions, and I accept responsibility for the content I approved.
  2. I understand tattooing risks include allergic reaction to pigments (which can occur years later), pigment migration or blowout, uneven healing, ink loss, raised or textured scarring, and changes in appearance over time from sun exposure, aging and skin changes.
  3. I understand that a tattoo is permanent, that removal is expensive, uncomfortable and often incomplete, and that variations in color and line as the tattoo heals are normal and not a defect in the work.
  4. I understand that if I have a condition such as eczema, psoriasis or keloid scarring, tattooing may trigger or worsen it at the site.

Piercing services

  1. I understand piercing risks include swelling, prolonged tenderness, migration or rejection of the jewelry, embedding, keloid or hypertrophic scarring, nerve irritation, and infection if aftercare is not followed.
  2. I accept the piercer's judgment on jewelry size, style and material for an initial piercing, and I understand that anatomy varies and that some placements may not be possible or advisable for me.
  3. I understand healing times vary from weeks to a year or more depending on placement, and that changing jewelry too early, sleeping on the piercing, or handling it with unwashed hands can ruin the result.
  4. I understand oral and genital piercings carry additional risks, including tooth and gum damage for oral placements, which the piercer will explain before proceeding.

Laser services

  1. I understand laser treatment risks include blistering, crusting, swelling, temporary or permanent lightening or darkening of the skin, scarring, incomplete ink clearance, and paradoxical darkening of some cosmetic inks.
  2. I understand that the number of sessions needed cannot be promised in advance and that some inks and skin types respond poorly or not at all. No degree of clearance is guaranteed.
  3. I agree to wear the eye protection provided during treatment, to disclose photosensitizing medications, and to keep the treated area protected from the sun as instructed before and after each session.
  4. I understand sessions must be spaced out to let skin recover, and that treating too soon increases the risk of damage.

Release

In consideration of the Studio performing the service I have requested, and to the fullest extent permitted by Arizona law, I release and hold harmless InkSquad Studio and its owners, artists, piercers, technicians and staff from all claims, demands and damages arising out of the inherent risks described above, out of my failure to follow aftercare instructions, or out of inaccurate or incomplete health information I provided. I understand this release does not apply to liability that cannot lawfully be released under Arizona law, including gross negligence or willful misconduct.

I have had the opportunity to ask questions about this form and the service, and my questions have been answered to my satisfaction. This release is governed by the laws of the State of Arizona, and it binds me, my heirs and my legal representatives.
