Client Acknowledgment Form
Every client completes and signs the shop's liability form before a service. The form is right below, so you can sign online before you arrive, or do it at the shop when you come in. Read the Service Terms & Conditions and the Service Liability & Release form first; completing the form confirms you have read and accept both.

Complete the Liability Form
This is the shop's liability form, hosted for InkSquad Studio. Fill it in and sign, and it lands in the shop's records, matched to your appointment when you come in. Minors receiving a piercing: a parent or legal guardian must still attend with ID and remain physically present for the procedure, as A.R.S. § 13-3721 requires.
